Notice of Availability of Form 1095-B
In accordance with the Paperwork Burden Reduction Act, MVECA will no longer automatically mail a paper copy of Form 1095-B, Health Coverage to all individuals for the 2025 tax year.
How to Request Your Form
You have the right to receive a copy of your Form 1095 upon request. To request a copy, please contact us using one of the following methods:

Timeline for Delivery
